YIWP will once again be publishing a Yizkor Book to be used during the Yizkor remembrance services throughout the year.
Please consider sponsoring the booklet to honor the memory of a loved one who has passed.
Donation is $18 Per Name
Please include on the form below: English name of deceased, Hebrew name of deceased (including mother and fathers name), relationship to deceased, and date of passing (Yahrtzeit).
Each form accepts 5 remembrances (names). If you are sponsoring for more than 5 remembrances, please complete an additional form.
To allow time for printing, all names must be submitted by AUGUST 29, 2018
